1. Fee/s:

* A $25.00 registration fee is due within 48 hours of registering for the designated training.  An email will be forwarded to confirm registration.  The remaining fee is due (24hours) after the EXAM day in electronic form. If the remaining fee is not paid within 24hours of the Food Service Sanitation Manager exam, the exam and license paperwork will be considered VOIDED and NOT filed with the Illinois Department of Public Health, regardless of the completion of the course.

Attendance Policy and 1st Day No
Shows:

Trainings begin promptly on designated day and time. All trainings are structured to meet or exceed the 15 hour requirement of the Chicago and Illinois Department of Public Health. Therefore, all trainings must be completed in full entirety. If attendance is broken once training begins, no refund will be honored unless instructors’ attendance is broken or any major acts of nature interrupt or prevent the training from continuing and/or concluding. If so, The Urban Food Safety Advocate (UFSA) will reassign participant to another UFSA hosted training session, within reasonable accommodations; at no additional charge or full refund.

3. Minimum Count-Cancellation:

If a training attendance does not meet the required (5) participant per training session, the participants will be contacted (10) business days prior to the training for a possible reschedule of another UFSA training (at no additional charge); *with a full registration refund.

4. Participant Cancellation:

*Must be received (10) businesses days prior to training; to receive a refund on registration fee.  There is a limit of one (1) rescheduling per attendee, within 3 months from date of payment. Any cancellations received after training, will result in a forfeiting of the proposed training. If any major acts of nature interrupt the training from continuing and/or concluding, The Urban Food Safety Advocate (UFSA) will reassign participant to another UFSA hosted training session, within reasonable accommodations; at no additional charge.

5. Disputes:

Any disputes regarding the training session may be subjected to binding arbitration if either I/WE or The Urban Food Safety Advocate so elects. If the UFSA elects to arbitrate any such disputes, I/WE will have no right to a jury trial for that dispute, and vice versa, and certain other consequences apply.

6. NOTE:

The Chicago and Illinois Department of Public Health (CDPH and IDPH) require a minimum of 15 contact hours of training. Therefore, no classes are to be missed. If the food service sanitation exam (given on the last day of training) is missed, the participant must retake the entire training, at full fee. A separate fee (from the CDPH $40 & IDPH $35) is needed to obtain each Food Service Sanitation Manager License upon successful completion of the exam.

7. FOOD SAFETY LICENSE:

We provide Food Safety Sanitation Training for the STATE OF ILLINOIS AND CITY OF CHICAGO.  The trainings OFFER the: 

FOOD SERVICE SANITATION MANAGERS LICENSE FOR THE:

  • STATE OF ILLINOIS

AND

  • CITY OF CHICAGO(RECIPROCITY)

The CITY OF CHICAGO AND STATE OF ILLINOIS REQUIRE A LICENSE NOT A CERTIFICATION!!!!
